Mediation request form
If you wish to open a file at the Université de Montréal Mediation Clinic, please complete the mediation request form below and send it to the following email address: cliniquemediation@droit.umontreal.ca. We will contact you to inform you of the status of your request. In order to open your file, we need to receive the mediation request form completed and signed by all participants involved in the conflict. In order for us to identify these participants, it is important to complete Section 2 of the form as many times as there are participants. If you can’t communicate with one of the participants, we can act as convening agent to propose the use of the mediation services of the clinic.
